Horizon South

Horizon South, a program of Horizon Services Inc., is fully licensed and certified by the State of California Department of Drug and Alcohol Programs to provide non-medical alcohol and drug detoxification and residential services for adult men. We have operated under contract with the Santa Clara County Health and Hospital System and the Department of Alcohol and Drug Services since February 1, 1991, providing combined non-medical detoxification and residential services in one facility.

Currently, Horizon South operates two separate programs: a ten-bed detoxification unit and a 31-bed residential program. Both programs are an integral part of the Santa Clara County Alcohol and Drug Managed Care System designed to provide a continuum of care for adults.

Horizon South programs provide services to men of ethnic and cultural diversity, men from the gay community, men with dual diagnosis and other disabilities, and the elderly. Horizon South is licensed for six (6) non-ambulatory beds for the mobility impaired and is wheelchair accessible. Parking for disabled persons is available near the main entrance.

Horizon South is also a referral source for men with substance abuse problems who have concurrent physical, mental health and other health issues. We are a primary referral source for men with substance abuse that have sustained head injury.

Detoxification

Horizon South operates a ten bed residential non-medical detoxification unit for adult men 18 years and older. Services are provided 24 hours a day with 24-hour monitoring. Services include complete assessment of alcohol and drug history, emerging withdrawal symptoms, medical referral if necessary, and structured monitoring using the Clinical Institute Withdrawal Assessment (CIWA-A) (ASAM Patient Placement Criteria). Client withdrawal symptoms are monitored and assessed every half-hour for the first 12 hours and as required thereafter until symptoms are under control.

Non-medical residential detoxification is appropriate when the client experiences physiological dysfunction during withdrawal from alcohol or drugs that is not severe enough to threaten life of significant body functions. Such clients must exhibit withdrawal effects from alcohol or drugs. Clients must be capable of making decisions on their own behalf and be able to care for themselves within the parameters of dysfunction usually associated with alcohol our drug withdrawal. The client does not require medications to facilitate withdrawal.

Entry into detox is through the managed care Gateway Program or by direct application to Horizon South. Information on clients entering detox including withdrawal status, recommended length if stay, and assessment for referral to ongoing services, is provided to the manages care system on a daily basis. The length of stay for detox is averaged between 4 to 6 days.

Clients in detox receive individual counseling, education in alcohol and drug addiction abuse, and referral and entitlement information. Within approximately 72 hours, an assessment process begins for referral to ongoing services. Clients are referred to either residential or outpatient services. Referral to outpatient services may include a referral to managed care transitional housing.

Residential

Horizon South operates a thirty-one bed residential treatment and recovery program for adult men 18 years and older. Services include assessment consistent with Santa Clara County managed care procedures and protocols using a system approved psychosocial assessment and the ASAM Patient Placement Criteria. (ASAM PPC-2). Information gathered includes alcohol and drug history, medical and mental health history, legal status, psychosocial evaluation, and other relevant data. 

Within 72 hours of intake, the client will be assigned a counselor and begin participation in the educational activities of the program. A formal orientation will provide the client with an understanding of managed care services available, program guidelines, and the process leading to stabilization.  Activities include education classes, group and individual counseling, relapse prevention information, and introduction to community based support groups such as AA, NA, and Rational Recovery.

Once the counselor assignment has been made, the development of an individualized treatment plan begins.  The treatment plan serves as a "roadmap" of strategies designed to address the specific treatment needs of the client.  this means counselor and client together identify the most pressing issues relative to the current treatment situation and fashion brief, measurable, and effective interventions that will not only effect stabilization, (the primary target), but serve as immediately useful tools for the client as he makes his way through the managed care continuum of care.  The individualized treatment plan is an "active" document, undergoing modifications, fine-tuning, and updating throughout the client's residential treatment at Horizon South.

Client stabilization occurs between 15 and 30 days. This period is designed to provide clients with insight into their alcohol or drug addiction or abuse.  Clients will experience a supportive environment and activities that emphasize alternate behaviors, enhance early sobriety and form the basis for sustaining clean and sober living.

Discharge planning begins 14 to 20 days prior to discharge and referral.  Clients are appropriately referred to on going managed care services, primarily outpatient placement consistent with their treatment needs.  Referral to transitional housing or other sober living environment is coordinated with the Department of Alcohol and Drug Services.

Horizon South provides services to men with special needs including dual diagnosis, head injury, HIV/AIDS, and physical and developmental disabilities.  We are wheelchair accessible, and licensed for six (6) non-ambulatory clients.  Special parking for disabled persons is available near the main entrance.

Services are provided in a modern, well-appointed home-like environment that is air-conditioned and well lighted.  Although a busy facility, the noise level is quiet to moderate.  The facility is staffed and monitored 24 hours daily.
