The Diversion Triage Center (DTC) is a uniquely designed program purposed with providing Jail Diversion to individuals at-risk of incarceration. The DTC provides services to adults ages 18+ who have been diverted from incarceration and who may be experiencing a mild-moderate mental health or substance use crisis that does not meet 5150 criteria. This program seeks to keep individuals out of jail and away from unnecessary hospitalization while providing ongoing support to and connections to appropriate community-based programs for increased health and independence.

Key Features:

  • Referrals: Law Enforcement Officers, Crisis Teams, CommunityBased Programs.
  • Client Workflow: Referral, Intake, Assessment, Optional Prescriber, Case Management, Initial and Planning, for the next 30-days, Discharge/Completion-Successful connection to ongoing resources.
  • Staffing: The clinically managed program is staffed across sites, with an Associate Director of Clinical Services, a Clinical Program Manager, a Prescribing Nurse, Forensic Peer Specialists, Mental Health (MH) and Substance Use Disorder (SUD) Case Managers that will providers to meet their needs.
  • Measurement Based Care: Evidence-based practice of assessment utilizing the patient’s perspective through patient-reported progress and outcomes.
