How Medi-Cal covers substance use treatment in California

In California, Medi-Cal covers substance use treatment through each county’s behavioral health system. Alameda, Santa Clara, and San Mateo counties all take part in the Drug Medi-Cal Organized Delivery System (DMC-ODS), a county-run network of care for eligible Medi-Cal members, organized around American Society of Addiction Medicine (ASAM) levels of care.

Your county and your clinical needs shape where care starts. Some Horizon programs take calls directly; others need a referral from a county access line or a county provider. The table below shows the route for each program.

County access lines

Some programs need a county referral first. Your county’s behavioral health line can help with screening and referrals for Medi-Cal treatment.

San Mateo County

How to start with Medi-Cal

Getting started takes one call. Here’s what usually happens.

1. Call the program, or contact us

If you're not sure where to start, call our main line and we'll help you choose.

2. Talk through your needs

Staff ask about your substance use, health, and goals, and whether you have Medi-Cal.

3. Get routed to the right level of care

If you need detox before residential treatment, or a county referral first, staff will explain that step.

4. Begin care

Staff walk you through the program and help you start your plan.

What to have ready (if you have it)

  • Your Medi-Cal card or member number
  • The county you live in
  • A phone number where we can reach you

Don’t have your Medi-Cal card, or not sure whether you’re enrolled? Call anyway. You don’t need every answer before you reach out.

Ready to talk?

We’ll help you figure out the right next step. There is no judgment here. Medi-Cal accepted.

If you don't have Medi-Cal

I have private insurance

Call the program or our main line. Horizon is a Medi-Cal and county-partnered provider, and staff will talk through your situation and the options available to you.

I don’t have any insurance

Call anyway. Staff can talk through your options, and your county’s behavioral health access line can help you learn whether you may qualify for Medi-Cal or county-funded treatment.

I live outside Alameda, Santa Clara, San Mateo, or Butte County

Medi-Cal treatment is organized by county, so start with your own county’s behavioral health access line. In New Mexico, the Gateway Medical Sobering Center in Albuquerque accepts referrals 24 hours a day.

Next step

For families, caseworkers, and referral partners

Family members, caseworkers, and partner agencies can call a program on someone’s behalf. Each program sets its own referral route, so use the table above to find the right one.

  • Families: you can call for a loved one. Programs such as Cronin House offer family counseling or family support when it’s appropriate.
  • County providers and hospitals: Cherry Hill accepts referrals from Alameda County providers; Mission Street requires a referral from an approved Santa Clara County provider.
  • Law enforcement and crisis teams: the Diversion Triage Center and Gateway Medical Sobering Center both accept referrals from law enforcement.
  • Consent and privacy: Horizon protects substance use treatment records under federal rules (42 CFR Part 2) and HIPAA, and we don’t sell client health information. Details are in our Notice of Privacy Practice.
