Yes. Horizon accepts Medi-Cal, and for eligible members care is often available at no cost. Call the program you’re interested in to talk through your coverage.
It depends on the program. Palm Avenue, Cronin House, Project Eden, and Butte County take calls directly. Mission Street needs a referral from an approved Santa Clara County provider, and the Diversion Triage Center takes referrals from law enforcement, crisis teams, and community-based programs.
No. Chrysalis is closed for renovation, and a new facility is scheduled to open in 2027. Women’s residential care is available now at Cronin House in Hayward, which is co-ed.
Yes. Family members, caseworkers, and partner agencies can call a program on someone’s behalf. Each program sets its own referral route.
